TALENT RELEASE FORM – YOUTH PARTICIPANTS
I hereby irrevocably give BAYCAT the right and permission to copyright and/or publish, reproduce or otherwise use my name, voice and likeness and/or written material, photographs and audio-visual about me or by me (collectively referred to as “Released Subject Matter”) for instruction, art, advertising or any other lawful purpose. BAYCAT will own any and all rights in said Released Subject Matter of me in perpetuity.
I hereby agree to relinquish all rights, title and interest I may have in the finished product or the advertising copy that may be used in connection therewith to BAYCAT, and waive all rights to any further compensation thereof. I hereby agree to indemnify and hold BAYCAT harmless against any claim of mine and/or of my heirs, executors or administrators, arising hereunder, which may hereafter be asserted against BAYCAT. This instrument constitutes the entire agreement, BAYCAT and persons below, and may be altered, amended or revoked only in writing signed by both parties.
By checking the box below, I acknowledge that I am a parent/legal guardian of the youth and give my permission to enter the contest. I have read and accepted the Official BAYCAT Represented Media Challenge Rules. I confirm that the entrant is eligible according to the contest rules.
